Flu Vaccine

Dear Parents/Carers,

This is a polite reminder that the flu vaccines are being held on the dates below, please return your child's form to school  with your indicated choice of yes or no as soon as possible giving us enough time to collate all the information.   
Please be aware even if you DO NOT require your child to have the vaccine you will need to complete the form with NO and return it to school, any forms not received will be called and chased by the nurses on the day.
